House File 571, signed into law Tuesday, May 19, gives a health practitioner or institution the right to refuse a service that contravenes their conscience and shields them from discrimination over their decision.

The law defines “conscience” as ethical, moral or religious beliefs or principles held by a health care provider or institution. It takes effect July 1.

[…] Rep. Austin Baeth, D-Des Moines, a practicing physician, countered that the policy would add another hurdle for patients to access to care, especially in rural areas where there already are fewer providers.

“There are no guardrails on what is conscience and what is just discrimination,” Baeth said. “This bill does not have a good purpose that is real. It only opens up for multiple harms and discrimination, and when that happens in the medical field, people go without lifesaving care.”

Unlike private clinics, federally qualified health centers are required by law to serve all patients regardless of their ability to pay, making them a critical safety net for low-income and uninsured residents in rural and urban areas. When one closes, there is typically no equivalent replacement—private providers can turn away patients that FQHCs cannot.

Iowa has also ranked last among all states and Washington D.C. for the number of state psychiatric beds per capita, according to the Treatment Advocacy Center.

Des Moines Public Schools have lost almost $47.5 million in state funding since Iowa’s private school voucher program was implemented before the 2023-24 school year, according to state data.

[…] Heading into the 2025-26 school year, the program opened to all Iowa families regardless of income—and 3,121 students living in the Des Moines Public school district participated, resulting in a loss of nearly $21 million in state funds.

So who’s the program helping? Analyses show that students who were already attending private school prior to qualifying for the program are the ones taking advantage of the program. In other words, their families were already planning to pay for private school tuition before being eligible for a voucher.

I do some work for an ethanol company and so if you’re able to join, I will give you $100, and for anyone that you recruit, an additional $25. No limit on referrals, so if someone recruits a group of 20 to show up, that’s $500.”

Republican elected officials argue that being a leader in tax cuts and “school choice” equal success for the state. Other metrics tell a more concerning story. Iowa has tepid population growth overall, with ongoing decline in rural areas. Medical care is difficult to find outside the state’s largest cities. Economic growth is similarly lukewarm, with Democrats repeatedly touting an analyst’s ranking of Iowa as in 50th place nationally. The state is using over $1 billion from reserve funds to balance the budget for the second straight year, as reduced tax revenue collides with expanded spending on private school tuition and Medicaid.

Iowa has the second-highest rate of new cancers nationally and is one of only three states with rising new cancer rates, according to this year’s Cancer in Iowa report. Possibly the most concerning: The rate of new cancers among young adults in the state — those from 20 to 39 years old — ranks second highest nationally, the report says.

“If you live in Iowa, your risk of being diagnosed with cancer is meaningfully higher than if you lived in almost any other state,” Deming said. “Cancer is now the second leading cause of death in Iowa” after heart disease.

The pesticides most heavily used in Iowa are glyphosate, acetochlor and atrazine — weedkillers that are associated with cancers that include colorectal, lung, kidney and non-Hodgkin lymphoma, the report said.

The European Union banned the use of atrazine and acetochlor, and the International Agency for Research on Cancer classified glyphosate as a probable human carcinogen. The U.S. Environmental Protection Agency has determined the pesticides are safe to use in America. Compared to Illinois, Kansas, Nebraska and Indiana — other ag-heavy Midwestern states — Iowa was the leading user of acetochlor and glyphosate from 1999 to 2019, the report said, with the Hawkeye state’s glyphosate use more than tripling over that time.

Rep. Josh Meggers, R-Grundy Center, who managed the bill on the floor, said the measure is “straightforward and narrowly tailored,” arguing it promotes consistency in state policy and improves public safety by ensuring drivers understand traffic laws “as written and posted.”

Democrats pushed back, warning the policy could lead to more unlicensed and uninsured drivers on Iowa roads. Rep. Jeff Cooling, D-Cedar Rapids, said there is no evidence that offering exams in multiple languages increases crash risk and warned the bill would instead push more people to drive without licenses or insurance.

The law’s opponents say the savings accounts have effectively subsidized private schools at the expense of public education.

Indeed, 99% of the private school students in Iowa are now using ESAs to pay for their schooling, and most already attended private school before the law passed, according to a Des Moines Register analysis.

Behind closed doors, secretively and quietly, Iowa Senate Republicans once again killed a bill that would improve, and possibly save, the lives of nursing home residents.

The bill would protect the right of loved ones of nursing home residents to install video cameras in their rooms. The camera would allow families near and far to check in on their moms, dads, grandparents or others to make sure they are well, safe, and receiving good care.

No one’s privacy would be violated. The cameras would be allowed only if the resident and loved ones—and a roommate if there was one—gave their express permission.

For loved ones, the cameras would ensure peace of mind. For facilities, they would ensure accountability for the quality of care being provided.

Republicans are responsible for this bill’s death each year. They have controlled the Iowa House and Senate every year the measure was buried.

“Leaving a violent relationship is already the most dangerous time for a victim; this bill could potentially force them to prove their abuse in court when they are possibly at their most terrified of the person who harms them,” Pingel said.

Green says the bill was made with those circumstances in mind and that this bill is part of an ongoing conversation.

“Yeah, would it be harder? Yes, I do think it would be harder," Green stated. “And so I want to investigate, you know, in those domestic abuse situations, could we provide some sort of more easy out for those situations?”

Tyler Coe is a divorce lawyer in the Des Moines area and says in his experience, his clients would be most hurt by the added court fees due to prolonged cases.

“Most folks do not have the funds to do that, and in my opinion, the money that they would pay to get a divorce under fault grounds will be better spent on their families,” he said.

Coe also raised concerns over the added challenge of proving abandonment, or abuse, as the bill currently requires.

Atlas Van Lines conducted its annual migration patterns study and finds Iowa ranks number 7 on the list of states with the highest percentage of residents moving out in 2025. The study was based on data from November 1st of 2024 to October 31st of last year. Atlas finds that during that time in Iowa, 43.6% of its moving vans were heading into the state, versus 56.4% percent heading out.

New rankings from LeapFrog put Iowa at 48th in the nation for hospital safety.

The nonprofit that focuses on patient safety looked at 30 Iowa hospitals that had sufficient publicly-available data to rank them on 32 measures of patient safety, including infection rates, surgery problems and staff communication and responsiveness. It’s the latest round of the group’s semi-annual ratings, which are released in the spring and fall.

Iowa landed at the bottom of its hospital safety list for fall 2025, along with North Dakota, Vermont and Wyoming. All four states had no hospitals that received an “A” rating.

“That’s not a new trend in Iowa. I did look at a few past rounds, and there’s been a lack of ‘A’ safety grades in the state,” said Katie Stewart, LeapFrog’s director of health care ratings.

Iowa’s economy shrank by 0.5% in 2024, making it the second-worst performing state in the country, according to new data from the U.S. Bureau of Economic Analysis. The contraction in real gross domestic product (GDP), adjusted for inflation, placed Iowa 49th among all states, ahead of only North Dakota.

The state also ranked 48th in personal income growth at 2.4%, ahead of only Nebraska and North Dakota, and had the second-slowest job growth nationwide in 2024 at -0.6 %, trailing only the U.S. Virgin Islands.

“Our biggest problem is still a weakness in our labor supply. We don’t have enough workers,” Orazem said. “Certainly if you look at the last 10 years or so, most of the attention paid on the political side has not been related to making Iowa a better place to live.”

“I already know that state employees aren’t allowed to strike. And if Iowa becomes a lot more hostile against worker protections, I could see a lot of people that get trained in trades and such leaving the state,” he said.

Other students mentioned a lack of amenities, the state’s abysmal mental health care system, and abortion rights getting chipped away.

Last December, Reynolds announced Iowa would not participate in this year’s Summer EBT for Children program. The program provides an extra $40 a month per child during the three months of school vacation to families whose kids qualify for free or reduced price meals at school. Food insecurity among school-age children spikes during summer months when they no longer have regular access to meals at school.

The $900,000 the governor allocated for the grant program amounted to 3.1 percent of the $29 million that would have been available through the Summer EBT program. According to the the nonprofit Iowa Hunger Coalition (IHC), the federal program would have covered 240,000 children in the state. In contrast, during the summer of 2023, the average number of children who ate on a daily basis at one of the summer meal sites the grant program was aimed at was 21,557. According to the governor’s office, the grant program resulted in the creation of a total of 61 new meal sites around the state this summer.

“This Book is Gay” is coming off the shelves at Northwest Junior High and other Iowa City Community School District libraries following a Twitter blast aimed at the Coralville school that administrators believe was linked to a pair of bomb threats.

The school had to be evacuated on consecutive days last week while police searched it. No explosive devices were found.

The Libs of TikTok Twitter account, which has 2 million followers and is known for its anti-LGBTQ stance, published a Tweet on March 21 that denounced the school for having the book, which it said promotes “gay sex and encouraged the use of sex apps.” It generated hundreds of responses, many of them graphically homophobic.

A copy of the amended bill was not immediately publicly available, but Sen. Dawn Driscoll, R-Williamsburg, said in the committee meeting that the new version “prohibits the Board of Regents from conducting research into the production or use of manufactured protein products.”

“Lab grown products are made in a petri dish and are not the same as the high-quality beef, pork, poultry, goat, lamb raised by hardworking Iowa farm families,” Driscoll said. “The taxpayer dollars should not be used to support these products over the real meat raised on Iowa farms.”
